ET General Surgery Conference seeks to encourage discussion, planning, and the exchange of ideas on the topics of education, research, and training across the many facets of the field of surgery. It is designed to benefit the attendees as a forum to introduce new technologies and new ideas which are leading to better, safer, and more efficient medical care. The following topics are being considered:Cervical cancer.
Cervical cancer is the second most common cancer among women in the United States. There is no single established risk factor for the development of cervical cancer, and the major risk factors are the presence of human papillomavirus (HPV) types 16 and 18, and a poor cytology. The most common presenting symptom is abnormal or "fecal-like" cervical bleeding. Transvaginal or transrectal sonography has become the most important initial diagnostic test for cervical cancer; a careful examination of the cervix with or without an endocervical curettage should be performed on all women with an HPV-positive, abnormal Papanicolaou smear. The management of the patient with cervical cancer depends on the stage of the tumor. In the early stages (FIGO stage 1A), hysterectomy and bilateral salpingo-oophorectomy with pelvic lymph node sampling is the treatment of choice. In the absence of metastasis, patients with lymph node involvement require concomitant chemotherapy. No other treatment is indicated for FIGO stage 1A or less, and chemotherapy is recommended in patients with metastatic disease. Radiation is also necessary to treat lymph node metastasis, and there is some indication that chemotherapy is more effective when given before radiation.
